Generic Brand Usual Dose
adefovir dipivoxil Hepsera Adults: 10mg once daily. Renal impairment: CrCl 30−49mL/min: 10mg every 48hrs; CrCl 10−29mL/min: 10mg every 72hrs; CrCl<10mL/min (on hemodialysis): 10mg every 7 days after dialysis; (not on hemodialysis): not recommended.
Children: Not recommended.
entecavir Baraclude* Adults: ≥16yrs: Take on empty stomach. Compensated liver disease: nucleoside-naïve: 0.5mg once daily; lamivudine-refractory or known lamivudine or telbivudine resistance mutations: 1mg once daily. Decompensated liver disease: 1mg once daily. Renal impairment (nucleoside-naive): CrCl 30−49mL/min: 0.25mg once daily or 0.5mg every 48hrs; CrCl 10−29mL/min: 0.15mg once daily or 0.5mg every 72hrs; CrCl<10mL/min or on hemodialysis: 0.05mg once daily or 0.5mg every 7 days; (lamivudine-refractory or decompensated liver disease): CrCl 30−49mL/min: 0.5mg once daily or 1mg every 48hrs; CrCl 10−29mL/min: 0.3mg once daily or 1mg every 72hrs; CrCl<10mL/min or on hemodialysis: 0.1mg once daily or 1mg every 7 days. If on hemodialysis day, give after session.
Children: <16yrs: Not recommended.
interferon alfa‑2b Intron A Adults: ≥18yrs: 5 million IU daily or 10million IU IM or SC three times weekly for 16wks.
Children: <1yr: Not established. 1–17yrs: 3million IU/m2 SC three times weekly for 1wk, then increase to 6million IU/m2 (max 10 million IU) three times weekly for a total of 16–24wks.
lamivudine Epivir‑HBV Adults: 100mg once daily. Renal impairment: CrCl 30−49mL/min: 100mg for 1st dose then 50mg once daily; CrCl 15−29mL/min: 100mg for 1st dose then 25mg once daily; CrCl 5−14mL/min: 35mg for 1st dose then 15mg once daily; CrCl<5mL/min: 35mg for 1st dose then 10mg once daily.
Children: <2yrs: Not established. 2−17yrs: 3mg/kg (max 100mg) once daily.
peginterferon alfa‑2a Pegasys Adults: 180mcg once weekly for 48wks. CrCl<30mL/min or ESRD on hemodialysis: 135mcg SC once weekly; may reduce to 90mcg once weekly if severe reactions develop; if intolerance persists, discontinue.
Children: <3yrs: Not established. ≥3yrs: 180mcg/1.73m2 x BSA SC once weekly for 48wks; max dose 180mcg. Maintain pediatric dose through duration of therapy in those who turn 18yrs.
tenofovir alafenamide Vemlidy Adults: ≥18yrs: Take with food. 25mg once daily. Concomitant carbamazepine: 50mg once daily.
Children: <18yrs: Not established.
tenofovir disoproxil fumarate Viread Adults: ≥12yrs (≥35kg): 300mg once daily. Renal impairment: CrCl 30−49mL/min: 300mg every 48hrs; CrCl 10−29mL/min: 300mg every 72−96hrs; hemodialysis: 300mg every 7 days or after a total of 12hrs of dialysis; CrCl<10mL/min: not recommended.
Children: <12yrs or <35kg: Not established.
NOTES

*For doses <0.5mg, use oral solution.
